Thank you for choosing Western New York ENT. We understand your time is valuable. In order for you to have the best possible experience here at our office, please be prepared with the following information when you come in for your appointment.

Please arrive 10 minutes prior to your scheduled appointment to give us time to verify and input all your information.

Office Policies

Payment:

Payment is required at the time services are rendered. Western, New York Ear, Nose & Throat, PC accepts cash, personal check, Visa, Mastercard, and Discover. All copayments, coinsurance, deductible down payments, and balances are payable at the reception desk upon check in.

Returned checks: A service charge of $15 is required for returned checks. If there is a past-due balance, any amount paid will be applied to the past due balance first.

Late payments:

Patients with an outstanding balance of 120 days or more may be discharged from the practice unless payment arrangements are made and honored. These accounts will be referred to a collection agency unless prior agreements are made with our billing department.

Appointment Time Policy:

When we schedule appointments, the needs of our patients are always taken into consideration. If you are up to 15 minutes late for your scheduled appointment, we may have to reschedule your appointment. We appreciate your timely arrival.

Appointment Policy:

Our office requires 24 hour cancellation notice PRIOR to your scheduled appointment time. If you fail to notify our office 24 hours prior to your scheduled appointment, you will be charged a $40.00 fee. Please take the time to call and cancel.

Co-Pay, Co-Insurance and Deductible Policy:

Copays, Coinsurance and Deductibles are collected at the time of service per your insurance coverage. Please be prepared to pay any cost share required by your insurance company at the time of your visit. We will collect any necessary fees until your annual/calendar year deductible is met. Failure to provide copayment or deductible at this time will result in rescheduling of your appointment.

New York State Red Flag Law, now requires us to have photo identification in our patients chart. Please bring New York State Drivers License or Photo Identification with you to your appointment.

Appointment Items Checklist

  • Health Insurance Card(s)
  • Photo ID  (Driver’s license or other State or Federal picture ID)
  • Insurance referral (if needed)
  • Co-pay, co-insurance and deductible (will be collected at time of service)
  • Completed forms (only if No Fault)
  • List of current medications you are taking (both prescription and over the counter)
  • List of medication allergies
  • Contact information for your preferred pharmacy as well as whether your insurance requires a 30 or 90 day supply of medication
  • If you have had an MRI, Sleep Study, or any other applicable testing, please bring the written report with you
  • If you have had a CT scan or X-ray, please bring the written report and images (preferably a CD)
  • Copies of records from referring doctor
  • Copy of any old hearing tests or audiograms
